NASA has completed an unprecedented chapter in human spaceflight, carrying out the first-ever medical evacuation from the International Space Station (ISS) after an astronaut experienced a serious in-orbit health issue that could not be adequately treated in microgravity.
The historic emergency return brought a four-member international crew safely back to Earth early Thursday morning, marking the first time NASA has activated its long-planned but never-before-used medical contingency procedures for astronauts aboard the orbiting laboratory.
The Crew-11 SpaceX Dragon capsule splashed down in the Pacific Ocean off the coast of San Diego, California, at approximately 3:41 a.m. Eastern Time, concluding a roughly 10-hour journey home after departing the ISS the previous day.
Emergency Decision Made After In-Orbit Medical Event
According to NASA officials, the decision to bring the astronauts home early was made after flight surgeons determined that one crew member was experiencing a medical condition serious enough to warrant treatment on Earth. While the astronaut was reported to be stable, doctors concluded that the condition could not be fully managed using the limited medical resources available aboard the ISS.
Out of respect for medical privacy, NASA declined to identify the affected crew member or disclose the nature of the condition. However, agency leadership emphasized that the return was executed out of an abundance of caution rather than an immediate life-threatening emergency.
NASA Administrator Jared Isaacman approved the evacuation, citing the importance of astronaut health and the need for further diagnostic care that could only be provided on the ground.
“This was a precautionary but necessary decision,” Isaacman said during a post-landing briefing. “Our priority is always the safety and well-being of our crew.”
Who Was Onboard Crew-11
The Crew-11 mission included astronauts from three space agencies, underscoring the international collaboration that defines operations aboard the ISS. Those returning to Earth were:

The crew had launched to the ISS on August 1, spending more than 100 days in orbit conducting scientific research, station maintenance, and technology demonstrations.
Fiery Re Entry and Ocean Splashdown
The spacecraft fired its engines at 2:50 a.m. ET to begin the deorbit burn, placing the capsule on a controlled trajectory back toward Earth. Shortly afterward, the Dragon capsule plunged into Earth’s atmosphere, enduring temperatures exceeding 3,500 degrees Fahrenheit as friction heated the exterior during re-entry.
As the spacecraft slowed, four parachutes deployed, guiding the capsule safely into the Pacific Ocean. Moments later, the capsule splashed down, bobbing gently on the water’s surface as recovery crews moved in.
Boats quickly surrounded the spacecraft, attaching lines to stabilize it while it was pulled toward a waiting recovery vessel. Once onboard, SpaceX ground teams began cooling the capsule and inspecting it for debris that may have adhered to the hatch during descent.
Immediate Medical Response at Sea
Medical personnel were among the first to access the capsule once it was secured. The hatch was opened at approximately 3:19 a.m., allowing the astronauts their first breath of fresh air since launching months earlier.
As is standard after long-duration space missions, each astronaut was assisted out of the capsule on a stretcher due to the effects of prolonged exposure to microgravity. After more than 100 days in space, astronauts often experience muscle weakness, balance issues, and cardiovascular adjustments that make walking immediately after landing difficult.
Mike Fincke was the first to emerge, followed by Zena Cardman, Oleg Platonov, and Kimiya Yui. Medical teams conducted initial health checks on the deck of the recovery ship before transferring the astronauts via helicopter and jet to NASA’s Johnson Space Center in Houston for further evaluation.
Crew-11 was scheduled to remain overnight at a medical facility near San Diego before continuing long-term recovery procedures.
NASA Confirms Astronaut Is Stable
During a live press briefing held roughly an hour after splashdown, NASA officials reassured the public that the crew member of concern was doing well.
NASA’s Chief Medical Officer, Dr. James Polk, said the astronaut had been in stable condition prior to the return and was never in immediate danger.
He also clarified that the medical issue was not related to a previously cancelled spacewalk scheduled for January 8, nor was it connected to any other operational activities aboard the station.
“This situation was isolated and handled exactly as our protocols dictate,” Polk said.
First Use of Long-Planned Evacuation Protocols
The evacuation followed NASA’s Spaceflight Human-System Standard, which outlines contingency return procedures if onboard medical capabilities are deemed insufficient. Although NASA’s statistical models have long predicted that such an event could occur approximately once every three years, this marked the first time the plan was ever activated.
Senior NASA official Amit Kshatriya praised the astronauts for their professionalism and training, noting that all crew members are extensively prepared to manage unexpected medical scenarios.
“This is exactly why we train,” Kshatriya said. “The crew performed exceptionally under challenging circumstances.”
ISS Operations Continue Without Interruption
Crew-11 had originally been scheduled to return after the arrival of Crew-12, which is now expected no earlier than February 15 aboard another SpaceX Dragon capsule.
Until then, the ISS will continue operating with a reduced crew. Maintenance and research responsibilities will be handled by remaining astronauts, including NASA astronaut Chris Williams, who arrived at the station aboard a Russian Soyuz spacecraft in November 2025.
NASA emphasized that the early return will not significantly disrupt ongoing scientific work aboard the station.
A Reminder of the Risks of Human Spaceflight
Orbiting approximately 250 miles above Earth, the ISS serves as a critical research platform for studying the effects of long-duration spaceflight on the human body. These studies are essential for future missions, including NASA’s plans to return astronauts to the Moon under the Artemis program and eventually send humans to Mars.
The medical evacuation highlights both the inherent risks of space exploration and the robust safeguards NASA has developed to protect its crews.
The ISS Nears the End of Its Lifespan
The International Space Station, which has been continuously occupied since November 2000, is expected to be decommissioned after 2030. NASA plans to gradually lower its orbit until the structure burns up in Earth’s atmosphere over a remote area of the Pacific Ocean known as Point Nemo, often referred to as a spacecraft graveyard.
