The NHS is set to introduce a new wave of smartphone-based health apps designed to support patients recovering from serious heart conditions, in a move experts hope will dramatically improve access to cardiac rehabilitation across England. The initiative follows growing concern that traditional, in-person rehabilitation programmes are failing to reach large sections of the population who could benefit most after experiencing life-threatening cardiac events.
Cardiac rehabilitation is widely recognised as one of the most effective ways to help patients recover following heart attacks, heart failure, and other cardiovascular emergencies. These programmes typically combine supervised exercise, education about heart health, emotional support, and lifestyle guidance aimed at reducing the risk of future complications. Studies consistently show that patients who complete cardiac rehabilitation are significantly less likely to be readmitted to hospital and have improved long-term survival rates.
Despite this evidence, participation in conventional rehabilitation programmes remains stubbornly low. NHS figures reveal that only around 13 per cent of people diagnosed with heart failure in England currently take part in cardiac rehabilitation. For patients recovering from heart attacks, uptake is also far below what clinicians would like to see.
Health leaders say this gap in participation represents a missed opportunity to save lives, reduce pressure on hospitals, and improve quality of life for thousands of people living with heart disease.
Barriers to Traditional Rehabilitation
One of the biggest challenges facing cardiac rehabilitation is accessibility. Traditional programmes are usually delivered in hospitals or community centres at fixed times, requiring patients to attend sessions in person. For many, this simply is not practical.
Women, younger patients, people from ethnic minority backgrounds, and those living in deprived areas are particularly underrepresented in rehab schemes. Work commitments, caring responsibilities, transport difficulties, language barriers, and anxiety about group-based exercise all contribute to low attendance.
Some patients also report feeling intimidated by clinical settings or worry that they will not be fit enough to participate, while others struggle with fatigue or mental health issues such as depression following a cardiac event.
Recognising these challenges, the NHS has increasingly looked to digital solutions as a way to modernise care and reach patients where they are.
Seven Apps Approved for NHS Use
In response, seven smartphone-based cardiac rehabilitation platforms have now been approved for use within the NHS by the National Institute for Health and Care Excellence (NICE), the body responsible for evaluating the safety, effectiveness and value for money of medical technologies.

These apps will be made available to eligible patients for an initial period of three years, during which time the NHS will collect data on how well they work in real-world settings. The aim is to determine whether digital rehabilitation can meaningfully increase participation and improve patient outcomes compared with traditional approaches.
What the Apps Offer Patients
While each platform has its own design and features, all seven apps provide core elements of cardiac rehabilitation. Patients are guided through personalised exercise programmes that are adapted to their fitness levels and medical needs. These routines can be completed at home, at a time that suits the user, reducing the pressure of attending scheduled sessions.
In addition to physical activity, the apps include educational content explaining cardiovascular disease, treatment options, warning signs to watch for, and ways to manage risk factors such as high blood pressure and cholesterol. Many also offer dietary guidance, medication reminders, and tools to track symptoms and progress.
Psychological and emotional support is another key component. Living with heart disease can be frightening, and anxiety or low mood is common after a cardiac event. Several of the platforms include mental wellbeing resources, peer support communities, and techniques to help users manage stress and rebuild confidence.
Health professionals can monitor patient engagement and progress remotely, allowing them to intervene if concerns arise or adapt programmes as needed.
A Potential Game-Changer for Cardiac Care
Dr Anastasia Chalkidou, HealthTech programme director at NICE, said the approval of the apps marks an important step forward in making cardiac rehabilitation more inclusive.
“These digital platforms offer real potential to transform how cardiac rehabilitation is offered, allowing care to be tailored to individual circumstances,” she said. “We know that traditional programmes are not reaching everyone who could benefit, and early data suggests that digital options could help close that gap.”
She added that while digital rehabilitation is not intended to replace face-to-face care entirely, it provides an important alternative for patients who might otherwise receive no rehabilitation at all.
Clinicians also believe that offering patients choice is crucial. Some may prefer in-person sessions, while others may feel more comfortable engaging through an app, particularly during the early stages of recovery.
Caution for Older and Vulnerable Patients
Despite optimism surrounding the new technology, NICE has stressed that digital rehabilitation will not be suitable for everyone. Older patients, or those who are unfamiliar with smartphones and digital tools, may struggle to use the apps effectively.
As a result, guidance states that every patient must undergo a full clinical assessment before being offered a digital rehabilitation platform. Healthcare professionals will need to consider factors such as digital literacy, cognitive ability, physical limitations, and overall health status to ensure the technology is appropriate.
The apps are intended to complement existing services rather than replace them, and patients who need hands-on supervision or additional support should still be referred to traditional programmes where possible.
Addressing a Growing Health Challenge
Cardiovascular disease remains one of the leading causes of death in the UK, placing enormous strain on the NHS. Hospital admissions related to heart conditions are costly, and repeat admissions are common among patients who do not receive adequate follow-up care.
By expanding access to rehabilitation, NHS leaders hope to reduce long-term healthcare costs while improving outcomes for patients. Digital tools, if proven effective, could play a major role in achieving this goal, particularly as the health service grapples with staff shortages and rising demand.
The three-year evaluation period will be critical in determining whether these apps deliver on their promise. Researchers will examine participation rates, patient satisfaction, clinical outcomes, and cost-effectiveness to decide whether digital cardiac rehabilitation should become a permanent feature of NHS care.
For many heart patients, however, the rollout already represents a welcome shift. Being able to recover at home, on their own terms, may make the difference between engaging with rehabilitation or missing out entirely.
As the NHS continues to adapt to changing patient needs and technological advances, the move towards app-based cardiac support signals a broader transformation in how long-term health conditions are managed — one that puts flexibility, accessibility, and patient empowerment at the centre of care.
